Primary malignancy of the pulmonary arteries

1-Leiomyosarcomas of the pulmonary arteries.

2-Usually seen in the main or central pulmonary or into the right ventricle.

3-Can extend into contralateral pulmonary artery simulating saddle embolus or into right ventricle.

4-Lung is frequently affected by metastases.

5-Secondary thrombo-embolic events are common with features similar to those of recurrent classic pulmonary embolism.

6-D.D. between this lesion and pulmonary embolism
Both have the same appearance in CT, but we can differentiate as follow
-Massive sarcoma of the pulmonary could not give a complaint as seen in the embolism.
-MRI can differentiate between both as embolus appears hyper intense in both T1 and T2(sub acute blood) and if we give contrast, no enhancement seen.While in case of sarcoma, it will seen hypo intense in T1 and hyper intense in T2 and shows enhancement after contrast injection.
